Sharing Down Syndrome Arizona is now accepting nominations for the 2012 Outstanding  Citizen with Down Syndrome and Outstanding Community Service Award recipients.  Each recipient will receive a plaque and public recognition.

Nomination forms are attached for your use.

Nominations must be received by noon on Friday, September 7, 2012 along with a photo of the nominee (4x6 landscape, please).

Nominations will NOT be accepted after that date and time.

The awards will be presented on Saturday, September 22, 2012 during the Walk for Down Syndrome event at Tempe Diablo Stadium.  A public announcement will be made in November.
